          Dentists call natural looking white fillings, posterior composites. Mercury silver fillings have the disadvantage of being metallic looking and turn black as they age.           Many brands of
white fillings materials are approved by the American Dental Association for use with in the back of the mouth.           Years ago, white fillings were not strong enough to replace much of a tooth, however, modern dental techniques have been developed to strengthen larger white fillings. Most white fillings are placed directly into a tooth and hardened right after the dentist prepares a tooth. In another white fillings technique, called indirect white fillings, the dentist make a model of your tooth, makes the white fillings on the model, and then inserts the white fillings into your tooth. The indirect technique allows the dentist to harden the white fillings more than if the white fillings had been placed directly into your tooth.

          White fillings also look great in the front of your mouth. White fillings can even be placed onto the surface of your tooth using a dental technique called bonding. Bonding allows dentists to mold beautiful smiles like artists mold clay statutes. The use of a light hardened white fillings allows dentist to mold, shape, and sculpt your tooth without worrying about time or having to rush through the white fillings procedure. After the dentist is sure the white fillings looks great, it is then hardened using a dental curing light. When the light is placed close to the white fillings material, the white fillings material begins to chemically change and harden. Dental curing lights have really improved the dentist's ability to use of white fillings materials to create beautiful smiles.

Click to enlarge Upper reconstruction in a phobic patient. The patient was a pretty, 33 year-old female. Treatment included porcelain fused to metal crown & bridge, root canal therapy on all abutments, root tip extractions, and facial bonding on both upper lateral incisors. The upper anterior root canal and crowns were first completed to show her how pretty her teeth could look and that it could be accomplished quickly and painlessly. Extractions of the hopeless teeth were all completed at one time and initiated early to allow time for healing before bridgework. Then root canal therapy was performed on the posterior abutments before their preparation for bridgework. The patient experienced minimal post-operative pain.

Click to enlarge Bonding Technique. After acid etching, apply All Bond 2 dentin primer to remove the smear layer. Then apply All Bond 2 dentin enamel bonding agent with dual cured catalyst. Bis-fil 2B self cured composite is next placed in the interproximal boxes and somewhat over the occlusal floor. It can be compressed into the interproximal boxes by condensing on preformed, already cured 2mm wide balls of Herculite XRV bonding material. Herculite XRV bonding material is added on top. Light curing should occur for 60 seconds on each the occlusal, buccal and lingual surfaces. A metal matrix with regular wood wedges work fine. Refine anatomy with a #8 round bur and finishing burs. Then use Fortify.
